Does anyone know if there is an entrance to Bull and Bush, or is it
through the tunnels only.

I think that it's tunnels only.

No.

Bull & Bush -- is that the one near Angel station? Or am I confusing
that with City Road, on which preliminary work started but was never
completed?

City Road was the one in City Road, which was open from 1901 to 1922.
Bull & Bush, a.k.a. North End, was never opened.
