There are lifts from platform to street at the Picc / District line
station. Exit towards the rear of your Picc Line train at Hammersmith
as there are two ticket halls at Hammersmith and the one to the rear
of eastbound trains is closer to the exit which takes you across the
H&C / Circle Line station.

True, though I don't know why people would change there rather than at
Farringdon, apart from the lack of step-free access at the latter.
Well, if you're changing between Thameslink and the deep level tube
lines (Northern/Victoria/Piccadilly), it'd be a bit daft to head to
Farringdon and then double-back via the sub-surface (Circle/Met/H&C)
route...
