Depends what caused it. Presumably you were over an hour late so
you'll get 50%, maybe 100% (if you were two hours late) of the return
ticket price back in vouchers

Or, I believe, from their website. You do need to have kept the
ticket(s) to send in with the form.
You're forgetting the double compensation FGW are currently offering -
I believe you're entitled currently to all your money back (in rail
vouchers) for delays in excess of an hour.
