Default Chelsea Harbour

That would be 1972-06-04 ;-)

Seriously, I think it appeared on the London Connections map in early
2005, along with Shepherds Bush (WLL). Not sure about the printed
timetable though.

It's in the winter 2004 - 2005 printed timetable.

Not sure about the one before, but not in the previous winter.
